Operations KPI Scorecard Template

A KPI scorecard should tell leadership what changed and what to do next—not just display numbers. This structure combines thresholds, ownership, trend, and a required action for any metric outside tolerance.

Threshold rule

Define Red / Amber / Green before the reporting period. If teams decide what “good” means after seeing the result, the scorecard becomes subjective.

Weekly review rule

Green KPIs should consume almost no meeting time. Amber and Red metrics need an owner, cause, corrective action, and due date.
